Important Service Delivery Notice: Jamie Van Auken, MA, Registered Marriage and Family Therapist Associate, provides all clinical therapy services exclusively at The Buddha's Medicine, 3310 SE Oak St, Portland, OR 97214. Path With Heart Therapy represents Jamie's therapeutic philosophy, approach, and professional identity. All appointments must be scheduled through The Buddha's Medicine's Practice Better EHR system. When you book services, you are entering into an agreement with The Buddha's Medicine for service delivery.
Definitions
"We," "us," or "our" refers to Jamie Van Auken, MA, Registered Marriage and Family Therapist Associate, E-RYT 500, operating as Path With Heart Therapy
"You" or "your" refers to the user or viewer of our Website
"Client" refers to individuals who engage our therapeutic services through The Buddha's Medicine

Therapeutic Services and Relationship
Service Provision
Jamie Van Auken provides therapy services as an independent contractor at The Buddha's Medicine. All clinical services, appointments, billing, and records management are handled through TBM's systems and policies. By scheduling services, you agree to TBM's policies, procedures, and terms of service.
Not a Medical Emergency Service

No Therapist-Client Relationship via Website
Use of this Website does not create a therapist-client relationship. A therapeutic relationship is established only through formal intake procedures at The Buddha's Medicine, including completion of required paperwork and an initial clinical session.
Email and website contact forms should not be used to communicate urgent or confidential clinical information. These communications are not secure and do not receive immediate attention.
Scope of Practice
Jamie Van Auken is a Registered Marriage and Family Therapist Associate licensed to practice in:

Services are provided within the scope of this license and in accordance with professional ethical standards set by the American Association for Marriage and Family Therapy (AAMFT).
In-person services are provided at The Buddha's Medicine in Portland, Oregon.
Telehealth services are available to residents of Oregon and Washington only, in compliance with state licensure requirements.
Client Responsibilities
By engaging our services, you agree to:
Provide accurate and complete information during intake and treatment
Attend scheduled appointments or provide advance notice of cancellations
Pay for services as agreed upon with The Buddha's Medicine
Follow agreed-upon treatment recommendations
Notify us of any changes in your condition or circumstances relevant to treatment
Respect professional boundaries
Confidentiality and Informed Consent
All therapeutic services are governed by HIPAA privacy regulations, Oregon state confidentiality laws (ORS Chapter 109), and Washington state confidentiality laws (RCW 70.02). Specific limitations to confidentiality, including mandatory reporting requirements under both Oregon and Washington law, are outlined in our Privacy Policy and clinical intake documents provided by The Buddha's Medicine.
Cancellation Policy
Cancellation policies are established by The Buddha's Medicine. Specific policies regarding appointment cancellations, late arrivals, and associated fees are provided during intake and are part of your service agreement with The Buddha's Medicine..
Scheduling and Appointments
All appointments must be scheduled through The Buddha's Medicine's Practice Better EHR system. Scheduling links on this Website direct to The Buddha's Medicine's scheduling platform. By using these links, you are interacting with The Buddha's Medicine's systems and agree to their terms of use.

Arbitration
If negotiation is unsuccessful, disputes shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association. Arbitration shall take place in Portland, Oregon. Each party shall bear its own costs and expenses, including attorney fees.
Class Action Waiver
You agree that any dispute resolution proceedings will be conducted only on an individual basis and not in a class, consolidated, or representative action.
Jurisdiction and Venue
For any disputes not subject to arbitration, you agree to submit to the exclusive jurisdiction of the state and federal courts located in Multnomah County, Oregon.
